Somatic and Mental Support

We have general practitioner supporters that assist the general practitioner with people in need of extra somatic and mental/GGZ support. 

  • Dionne Tempels, Kim Cook and Marloes Timmerman hold independent consultation hours as general practice supporter.

 

Kim Cook

  • mainly assists patients with Diabetes, COPD, Asthma and Cardiovascular diseases.

Dionne Tempels

  • mainly assists patients with Diabetes, COPD, Asthma, Cardiovascular diseases and Elderly care.

Kim Cook and Dionne Tempels:  work together with Frisian care-chain (see more information on ketenzorgfriesland.nl).

 

Marloes Timemrman

  • guides patients with psycho-social problems.
